The OP-1 form is also used when a carrier or broker needs to reinstate previously granted but inactive operating authority. If authority has been revoked—commonly due to lapsed insurance filings, missing BOC-3, or other compliance issues—the carrier must submit an OP-1 Reinstatement request to the FMCSA.

Reinstatement through the OP-1 process confirms that the business is restoring all required information, including company details, the type of authority being reinstated, insurance documentation, and process agent filings. Once the FMCSA verifies these requirements and processes the request, the operating authority is reactivated, allowing the carrier or broker to resume lawful interstate operations.
